•Three hour clinical interview

•Brief history of patient

•History of eating behaviors

•History of weight issues and weight loss attempts

•Current and/or history of abuse

•Disordered eating behaviors

•History of exercise and physical activity

•Five axis diagnosis

•Current stressors

•Ability to cope with stressors

•Current and previous psychological problems and/or treatment

•Problems of daily living

•Current and previous chemical dependency

•Family history of chemical dependency and/or psychological problems

•Suicidal ideation and/or attempts

•Realistic expectations and understanding of surgery

•Mental status

•Current support system

•Special needs

•Ability to follow preoperative instructions and postoperative behavioral changes

If you are considering bariatric surgery (commonly known as weight loss surgery), it is important to educate yourself regarding the potential risks and benefits of surgery.  There are many resources available to help you make this life altering decision.  If you would like help with resources please contact my office for further information.  For those individuals who meet the criteria for bariatric surgery a psychological assessment is required before surgery.  You may ask your surgeon for referrals or contact my office if you are in need of a psychological assessment for bariatric surgery.
